Job summary

Temporary GP Contract to provide additional clinical sessions 4-5 sessions, Monday to Wednesday. Start date to be confirmed. End date 31 March 2027.

Newly qualified GPs are very welcome to apply.

  • Our clinical team comprises four partners and three salaried GPs, plus 4 ANPs
  • Excellent multidisciplinary team, with full skill-mix, including clinical pharmacist and physiotherapist.
  • Supportive admin team, keeping Docman, bloods and other admin to a minimum for clinicians.
  • Very low numbers of home visits for GPs we have two visiting paramedics who do the vast majority. We have no nursing or care home patients, so this keeps visits very low.
  • Full secretarial service no need to type your own letters!
  • Supportive environment with regular clinical and MDT meetings; in-house training events.
  • Member of Central Chester PCN, a collaboration between Chesters city-centre practices.

Main duties of the job

To act as an autonomous practitioner, utilizing knowledge and skills to provide care, treatment and management of acute and chronic health care problems to the whole practice population.

Work in a primary care setting in practice and visiting patients at home.

To provide health promotion and disease prevention advice to the practice population.

Provide evidence-based care using appropriate national and local guidelines.

Act in collaboration with colleagues from within and outside the practice as a member of the multidisciplinary team.

Refer patients to other providers of care as appropriate.

Engage in and actively encourage training and development of self and other staff.

Garden Lane Medical Centre is a busy Chester City Centre Practice with a friendly team of 7 doctors, 6 nurses, healthcare assistant, GP Assistant and phlebotomist. There is a large, extremely helpful team of administration staff.

Job responsibilities

Principle Responsibilities

Clinical

Exercise a high degree of professional autonomy and critical judgement. Make professionally autonomous decisions utilizing knowledge and skills gained via study at Degree and post graduate levels and lengthy experience in a variety of settings.

Proactively assess patients with previously undiagnosed and undifferentiated health care problems.

Assess health care needs, screen patients for risk factors and disease and provide health promotion advice.

Treat patients utilizing highly developed knowledge and skills, or refer to a range of primary and secondary care services as appropriate.

Order and interpret tests and investigations required for diagnosis and management of patients.

Review and monitor care plans and therapeutic interventions appropriate to patient need and in accordance with evidence based practice and national and local guidelines.

Prescribe in an independent and supplementary role and/or supply and administer medicines within competency as appropriate.

Engage with other members of the practice and multidisciplinary team to develop effective, evidence-based care for the practice population.

Contribute to the development of joint standards of care, clinical guidelines and management pathways for utilization within practice.

Maintain a high standard of professional care at all times in accordance with the GMC Code

Work as part of a team to contribute to achievement in the Quality and Outcomes framework.

Training and Education

Maintain own professional and clinical expertise via participation in clinical activities, professional updating and clinical governance activities.

Contribute to and maintain clear systems for recording and retrieving information in concordance with local and national data protection strategies.

Utilize appropriate data collection and storage systems i.e. electronic and written to collect and store patient data.

Ability to utilize practice systems and procedures as appropriate.

Counsel and support colleagues on professional matters.

Contribute to and maintain a culture within the organisation which promotes confidentiality and complies with professional, local and national requirements for data protection.

Utilize appropriate interpretation services as required i.e Language Line.

Research and Audit

Initiate and participate in clinical audit, encourage a practice-wide approach across the whole team, alerting colleagues to findings and the implication of those findings.

Evaluate the outcomes of clinical audit, applying findings to own and the teams practice, leading improvements in practice as the result of audit.

Contribute to the evaluation of relevant research findings to further develop the practice services.

Identify key areas for research/audit; develop appropriate methodology and supervise research outcomes and analysis of findings

Contribute to the development, dissemination, implementation and monitoring of clinical guidelines and care pathways.

Assist the practice manager in shaping and developing services utilizing recent evidence and best practice which reflects the needs of service users, the local health economy, and which responds to local and national health priorities.

Evaluate and audit the clinical practice of self and colleagues by the application of a broad range of valid and reliable methods to ensure competence and continuous quality improvements in both practice and service.

Promote opportunity for health gain by seeking the involvement of service users in opinion seeking activities designed to obtain service user views and promote health living.

Leadership- personal and people development

Contribute to the creation, maintenance and enhancing effective multidisciplinary team working within the practice and across the local health economy.

Delegate responsibility for safe storage, stock rotation and disposal of vaccines and drugs and oversee the monitoring processes in adherence with practice policies.
