Main duties of the job

Working as part of a multidisciplinary team, you will provide high-quality, patient-centred care to a diverse practice population whilst contributing to the ongoing development of our services.

You will play an active role within your practice team, working closely with GPs, Nursing Teams, Pharmacists, Advanced Practitioners and wider healthcare professionals to deliver safe, effective and compassionate care.

Job responsibilities

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting. They must act within professional boundaries and provide the highest quality of holistic care for all patients. A successful candidate will be working alongside a friendly, hardworking team of general practitioners, nursing colleagues, admin and reception staff in the care of practice patients.

Job responsibilities

The post-holder will need to ensure they are readily available to work alongside the Practice timetable, undertaking a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries.

Effectively manage paperwork and correspondence which need to be completed within a timely fashion.

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within organisation.

Assessing the health care needs of patients with undifferentiated and undiagnosed problems.

Screening patients for disease risk factors and early signs of illness.

In consultation with patients and in line with current Practice disease management protocols, developing care plans for health.

Providing counselling and health education.

Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.

Recording clear and contemporaneous consultation notes to agreed standards.

Collecting data for audit purposes.

Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible).

Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate.

In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

File abnormal blood tests, as well as those for absent GPs include microbiology as these arent marked abnormal if positive

File blood test for absent GPs in next 2 working days

Deal with ALL DOCTORS tasks.

Complete scanned documents/workflow ALL DOCTORS group including necessary referrals/etc

Awareness of and compliance with all relevant Practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safe

A commitment to life-long learning and audit to ensure evidence-based best practice.

Contributing to evaluation/ audit and clinical standard setting within the organisation

Computer data and patient records entry, allocation and collation, processing and recording information in accordance with practice procedures

Contributing to the development of computer-based patient records.

Contributing to the summarising of patient records and Read-Coding patient data.

Initiating contact with and responding to requests from patients, other team members and associated healthcare professionals and providers.

Attending training and events organised by the Practice or other agencies, where appropriate.

Helping to keep all practice areas tidy and free from obstructions and clutter.

Helping to maintain a positive, patient focussed culture.

Helping to maintain a positive, supportive culture across the whole of the practice team.

Filing pathology results and actioning related clinical work.

Filing of scanned documents/ workflow and the actioning of clinical work related to them.

Authorisation of acute and repeat prescriptions.

SystmOne Tasks and actioning related clinical work.

Completion of paper documents e.g. child protection forms and insurance documents.

Occasional home visits.

During busy periods including holidays & sickness you may be requested to see additional patients.

Attendance of clinical meetings at site and PCN level.

Duty Doctor Responsibilities

Urgent telephone appointments.

Urgent Tasks.

Limited pre-booked telephone appointments to increase telephone capacity and for continuity (limited on busiest days, Monday and Friday).

Supervise/support urgent/same day queries pharmacy team, NPs, nursing team, frailty team

Supervise FY2, NPs.

Assist with triage/allocation of home visits Paramedics triage from 8.30-5.30.

See face to face appointments if urgent and all other appointments taken, often after paramedic triaged patient.

Patient services

To provide medication review services to patients via clinics in the practice, domiciliary visits and in residential and nursing homes, and to deliver pharmaceutical care plans that maximise cost-effective prescribing and improve the quality of patient care.

To present at patient group meetings or other appropriate events to give advice on the appropriate use of medicines.

To produce patient information leaflets and posters and run medicine awareness projects throughout the year.

Assist Partners with the appropriate monitoring and management of their prescribing budgets.

To prepare evidence based resources and information to support the medicine management team and all other relevant health professionals in the implementation of rational cost-effective prescribing.

To help plan, develop and support the introduction of new working processes within the practice to optimise the quality of prescribing.
