RNLD - Care Home Health Facilitator

Job overview

The post holder will support residential care home staff to positively manage and meet the health needs of individuals with a Learning Disability within the Croydon Area.

The post holder will work collaboratively to ensure people living in care homes have access to enhanced primary care and to specialist services and maintain their independence as far as possible by reducing, delaying or preventing the need for additional health and social care services

Main duties of the job

• Support training and education
• Liaise with GP services to formulate and implement Urgent Care Planning in line with current guidance. Advocate for residents.
• To work with people who engage in behaviors that challenge (e.g. physical aggression to others, self-injurious behaviors etc.)
• Support staff with MCA/BI decision making with GP services/involved health professionals and families.
• Support care homes hospital admission and complex discharge planning.
• Ensure that care home staff feel supported and included as part of the MDT and integrated professional network
• Provide regular reports for LeDeR via the LAC and steering group.
• To contribute to multi-disciplinary planning and meetings
