Job overview

Respiratory/Oxygen Practitioner North Powys Band 6, Full time. 37.5 hours.

Role open to registered nurse or physiotherapist.

Provide knowledge, skills and experience in respiratory and oxygen practice, and act as a resource to advise, educate and support those involved in the care of people with respiratory disease, both in the primary and secondary care setting.

Provide specialist complex assessment, advice and education via Community Hospital specialist led clinics and on home visits to all patients requiring respiratory/oxygen assessment.

Provide safe, high-quality care to patients by implementation of policies/standards and guidance produced both nationally and locally, these will include from NMC, HCPC Welsh Government, Powys Teaching Health Board, and others.

Main duties of the job

Assess, plan, implement and evaluate the physical, psychological and social needs of Respiratory and Oxygen Therapy patients, initiating therapeutic treatments in collaboration with patients, carers and other health and social care professionals.

Responsible and accountable for an identified selected case load, working with the patient and the MDT to provide specialist respiratory and oxygen assessment, formulation of treatment plans and follow up.

Undertake and monitor patient assessment, care planning, intervention and evaluation in partnership with the patients and their carers, working collaboratively with the multi-disciplinary/multi agency team.

Detailed job description and main responsibilities

Plan with the patient/team a discharge plan that is followed through for each patient, involving carers, community services and other agencies when appropriate.

Support the implementation, with the Team Lead, the PTHB Strategy for Nursing and Midwifery or Therapy Strategy, and the delivery of respiratory and oxygen assessment in line with National Guidelines.

Work collaboratively within the Multi-Disciplinary Team and with other health care professionals to ensure a consistently high quality and equitable service throughout the Health Board.

Act in an advisory capacity, using clinical knowledge to inform decision making regarding patient care.

Support local initiatives at removing interagency barriers and maintain excellent professional links with health and social care providers to promote effective communication and joint working practices.

Work autonomously, making clinical decisions within own competency and ensuring effective caseload management continuously reprioritising to meet the changing clinical demands of the service.

Take appropriate action to ensure any clinical risks are identified and minimised.

Represent the team on working groups and committees.

Support the Team Lead in the efficient use of resources.

Participate in the care and maintenance of equipment and report faults within own area of responsibility.

Responsible for organising own workload within the requirements of the specialist team activities and work plan.

Be flexible in working patterns, providing support to neighbouring localities as required in support of colleagues and maintaining standards of patient care.
