Job overview

To work as an advanced practitioner and lead the King’s element of the KHP/Lambeth and Southwark Home Oxygen Assessment and Review service alongside counterparts at Guy’s and St Thomas’s Hospital.

To prioritise, assess, treat and manage own caseload of patients as an autonomous practitioner, including the maintenance of comprehensive records.

To work across both acute and primary care providing seamless care to a group of identified patients.

To support the COPD and asthma services as well as other relevant services as appropriate (including cardiology, palliative care, neurology, gerontology and others)

To ensure that patients and their families are fully informed about  oxygen therapy and supported to be active partners in the care, by providing holistic care.

To establish clear lines of communication within the specialty team and GPs to ensure cohesive multidisciplinary care for patients.

To undertake an evidence-based projects to further the clinical practice of self and the team, and to enhance patient care.

Detailed job description and main responsibilities

To assist in the delivery of the KHP/Lambeth and Southwark Home Oxygen Assessment and Review Service for people with needs in both hospital and community settings in line with local and national guidance and protocols.

To provide specialist respiratory advice and support to patients, their families and other healthcare professionals within the Trust and community settings as required. To ensure continuity of a high standard of evidence based, safe, effective patient centred care, assessing health, health related and nursing needs of patients, their families and other carers by identifying and initiating appropriate steps for effective care. This can include:

• Running oxygen clinics (hospital and community) and undertaking home visits Ensuring patients’ medical care is optimised Identifying and treating tobacco dependence Prescribing medicines and treatments (if non-medical prescriber) Performing arterial and ear lobe capillary blood gases as per protocol Ordering diagnostic tests) Agreeing personalised care plans relating to home oxygen with patients and carers and sharing these with relevant stakeholders as appropriate Admitting and discharging patients for specified conditions and within agreed protocols To accept joint responsibility for the oxygen caseload, within Lambeth and Southwark with Guys and St Thomas ’Hospital ensuring all parties have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations made ensuring clear documentation in the patient’s records and hand held records. To arrange follow-up patients as appropriate and ensure all relevant information is available to enable patients to make an informed choice about their treatment. To attend and play an active role in MDT meetings ensuring all patients are represented. To deliver a seamless service through the development of enhanced multidisciplinary team (MDT) processes and communication to ensure continuity of care. To participate in the identification and development of clinical protocols and strategies to enhance both the continuity and standard of specialist care whilst ensuring equity of access in the service. To participate in the process of operational policies review annually in accordance with national standards. To ensure all care is given in accordance with agreed protocols. To work with key staff members to maintain nurse-led assessment; providing detailed baseline patient assessment and education and support to patients and their families. To liaise with ward staff to ensure appropriate and timely discharge/follow up arrangement in place for patients. To work in the community in patients’ own homes as required under the integrated respiratory service. To maintain a suitable mechanism for MDT referrals to the service To work with the MDT to develop, implement and evaluate integrated care pathways and systems of MDT documentation. To actively involve service users in providing feedback of their experience of the current service and suggestions for improvements. To participate in relevant for a at Division, Trust or CCG level to represent the service perspective.
