You will work alongside the Nurse Practitioner, Renal F Floor Team, Transplant and Living Donor Coordination Teams, and Surgical Team to support patients throughout their transplant journey.
This is a varied and rewarding role that combines specialist clinical nursing, patient education, care coordination and holistic support across both the inpatient and outpatient settings.

You will provide specialist nursing support to renal transplant recipients during the immediate and early post-transplant period, working closely with the Renal F Floor Team. This will include:
Providing specialist nursing assessment, advice and support to new renal transplant recipients.
Delivering individualised education to patients and their families/carers. Participating in ward rounds (RUF) and supporting patients throughout their inpatient stay.
Reviewing patients in the outpatient setting following discharge, until they transition to the main renal transplant clinic pathway.
Reinforcing education around medicines, adherence, recognition of complications and effective self-management.
Supporting patients to develop the knowledge, skills and confidence required to manage their health following transplantation.
Identifying psychological, emotional and social needs and facilitating appropriate support or referral to services including renal psychology and renal social work.
Signposting patients to relevant patient support organisations, including the Sheffield Kidney Patients Association (SAKA).
You will also work collaboratively with the Renal and Living Donor Transplant Coordination Team.
