Job overview

An exciting opportunity has arisen to join the Transplant Specialist Nurse team. The post holder should have extensive knowledge of renal nursing, excellent communication skills, a passion for delivering high quality care and be able to work collaboratively within the team. Qualifications in prescribing and physical examination are desirable, but this could be a band 6 training post until band 7 competencies are achieved, and full support will be given to attain this.

Main duties of the job

Manage nurse-led face to face and telephone clinics. Manage the medication for this group of patients in conjunction with MDT discussion and according to local and national policies, assessing . Participate in research and lead in clinical audit. Participating in networking at local and national conferences. Deliver training for colleagues, support students on placement within the transplant team. Lead in the follow- up and risk assessment of any patient who has had any Health Acquired Infections, or complications post transplant such as steroid induced diabetes.

Detailed job description and main responsibilities

The position involves co-ordinating the care for a caseload of post transplant patients. Providing education, advice and support to patients and their families. Reviewing on each assessment, in conjunction with patient, fluid balance, blood pressure, nutritional status, sleep patterns and other concerns the patient may have. This involves assessing and interpreting complex medical and nursing needs, triaging and taking appropriate action on findings, and advising GP of any changes.
