Job overview

The registered nurse will work within the Urology Clinical Nurse Specialist team and under the supervision of the senior CNS will    Undertake nurse led follow up clinics for patients post treatment for prostate cancer, assessing patient’s needs, implementing and  evaluating care. Communicate any changes to care plan to the wider MDT and primary care.  Support and manage the care of patients on self-management and remote follow up of prostate cancer

Act as the point of contact and named nurse for patients on self-management follow up.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Main duties of the job

Develop and provide patient education to ensure patients are fully informed about the prostate cancer self-management system/process.  Holistically assess patient’s needs, developing and evaluation plans of care.  Mange the caseload of prostate cancer follow up patients
