Job summary

The post holder duties shall be :

To co-ordinate the delivery of a mouth care improvement program across targeted ward areas within the Acute Trust sites.

Co-ordination of the programme shall be within agreed timescales & milestones to define & deliver objectives specific to the mouth care needs of inpatients.

Undertaking specific local audits on various patient focused themes.

Deliver face to face training to clinical staff and bedside coaching.

Working collaboratively with nursing teams and MDT staff members.

Supporting local policies and procedures in relation to mouth care. introducing mouth care assessment and recording tools.

Supporting the embedding of mouth care documentation into the Electronic Patient Record (EPR).

Produce monthly NHSE KPI activity reports.

Contribute case studies and patient stories.

Supporting collection of denture loss incident data

Participate in staff feedback and focus groups.

Maintain training attendance records and evaluation forms.

The moving, carrying and transportation by car of stock and resources on a regular basis to a variety of sites.

Share responsibility for the security and stock of resources and equipment

Main duties of the job

Reviewing and making recommendations regarding guidance/policies in relation to oral health improvement/Mouth care.

To influence in-patients & staff attitudes and encouraging positive behaviour change.

To create an awareness of the importance of good oral health and adopting healthy lifestyle approaches within the scope of the project.

To be responsible for the recording of data, using manual and IT systems for the evaluation and monitoring of service activity and development.

Support the delivery of staff focus groups, nursing staff surveys, inpatient mouth care experience questionnaires.

To work with, support and offer specialist training for other professionals in the delivery of the mouth care matters programme principals.

Represent the Trust in National Campaigns, such as Smile month. Mouth cancer action month and world oral health day.

Work with ward teams to develop a local mouth care network..

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
