The Role

The post holder will be a Registered Nurse who will support the community nursing service in the delivery of the highest quality of health care and will be responsible and accountable for providing skilled nursing interventions as appropriate. The post holder will ensure a person centred approach to care delivery which is safe and effective to ensure and maintain quality strategy. The post holder will also be expected to provide an accessible, appropriate, high quality and effective care service to all age groups in the community, by working in partnership with patients, care providers and other agencies.

The working pattern for this role is - rotational/nightshift

In this key role, you will:

  • The post holder is responsible and accountable for assessing, planning and implementation of programmes of care. These will be developed in partnership with patients, family, carers and significant others, and will be evidence based taking into consideration the lifestyle, race, gender and cultural background, and factors which impact on physical, mental or emotional health.
  • Effectively deliver Community nursing practice and duties as detailed in relevant NHS Lanarkshire policies , Procedures and other relevant guidance e.g. tissue viability, infection control, terminal care . and be aware of their responsibility in respect of public protection.
  • Participation in Clinical Governance processes in all areas of Community nursing practice, including the involvement of patients/carers, to ensure continuous practice improvement.
  • Demonstrate a clear understanding of organisational goals

What You'll Bring:

  • Registered Nurse with valid NMC Registration
  • The post holder will be required to demonstrate excellent team working skills with the ability to work using own initiative
  • Effective listening, communication and interpersonal skills
  • Ability to work with people and as part of a multidisciplinary team

A driving licence is required; this must be a full UK/EU/EEA licence.

From 1 April 2026, the Agenda for Change full-time working week will reduce from 37 hours to 36 hours. Part-time working hours will be reduced on a pro-rata basis. A corresponding increase in the hourly rate will apply, ensuring that overall pay remains unchanged.
