A 37.5 hour per week (full time), Band 6 RMHN post has become available in Brighton & Hove Community Rehabilitation Service, for an experienced practitioner able to care coordinate a case load of service users with complex mental health needs in the community. This post is open to those with a professional qualification in Nursing.

We are a well-staffed multi-disciplinary team made up of Social Workers, Nurses, Occupational Therapists, Psychologists, Psychiatrists, Support Workers, Peer Support Workers and a Pharmacy Technician. We operate as a team within a creative and flexible approach and provide shared responsibility of risk management to support our service users who have a primary diagnosis of psychosis. We run groups for service users and has a biopsychosocial approach to support and we are keen to find a dynamic individual to join the team.

Main duties of the job

CRS Lead Practitioners will have significant experience of working with individuals with complex needs, including severe & enduring mental health issues, substance misuse problems, forensic issues and housing difficulties. The ability to work as an integral part of a large multi-disciplinary team which operates a whole team approach is essential. The team works with people who have had unsuccessful working relationships with mental health services in the past and are difficult to engage as well as those who have more clearly identified rehabilitation goals.

The team is highly regarded, well-resourced and mutually supportive with a commitment to providing high quality, flexible, creative, service user led support. The service operates seven days per week, and so some working at weekends and bank holidays is expected.

Detailed job description and main responsibilities

This exciting opportunity will offer you the opportunity develop and grow in your chosen career and in return you will be part of a dynamic team receiving ongoing support and supervision and the ability to work alongside a skilled Multi-disciplinary team. CRS operates a 7-day week, so you will be required to undertake a flexible approach to working hours. Our typical working hours are Mon-Fri 9-5 but different shift patterns can be negotiated if needed.

S75 agreement:

Community Mental Health Services in Brighton & Hove has a well-established Section 75 (S.75) agreement in place. This agreement contributes to the improvement in mental health and wellbeing and results in easier access to a more responsive mental health service.

The S.75 agreement between Brighton and Hove City Council and Sussex Partnership Foundation Trust has been in place for many years and has meant several Social Care staff have been co-located within the Trust to deliver statutory duties.  This way of working in close partnership has been valued by both organisations, and by those that use our services, and this new agreement seeks to maintain and strengthen this collaborative way of working.

The S.75 agreement serves as a significant tool in achieving the ambitions to transform adults and older adults’ community mental health services and reduce health inequalities.

In practice, Sussex Partnership NHS Foundation Trust and Brighton and Hove City Council delegate tasks to each organisation to ensure a joined-up health and social care offer to those that use our services. Relevant training and support will be provided when acting on behalf of the other organisation and it also means that you're able to access a number of other training, learning and development opportunities from both organisations.
