We have an exciting and rare opportunity for you to join our team as a Quality and Clinical Excellence Facilitator
This is a new post that will support the Quality and Clinical Excellence Team to drive and sustain improvements across the Trust.
You will work alongside our Clinical Leads for Quality to support a wide range of portfolios, including Deteriorating Patient, Nutrition & Hydration, Documentation, Falls, Deconditioning and Dementia.
We will provide training and education to fully equip you with all aspects of the job role.
Due to the nature of the role we are looking for a candidate with substantial NHS experience.
The role will require cross site working and so it is essential that you can travel across all Shropshire Community bases

Main duties of the job
The post holder will play a key role as a member of the Quality and Clinical Excellence Team, working across all clinical areas to improve the quality of care in all key nursing and patient safety domains. The postholder will be pivotal in progressing work around CQUIN achievement and ensuring the quality of care is sustained and /or improved as a result.
The post holder must have proven experience in implementing quality improvement initiatives and of engaging with the multi-disciplinary team to develop and implement mutually driven schemes. This includes learning from patient experience, adverse incidents and complaints.
A key remit of the role is to help drive and sustain quality improvements. This post holder will facilitate and co-ordinate a portfolio of quality improvement work across the Trust supporting the Trust’s delivery improvements, this includes but is not limited to improvement initiatives linked to falls and deconditioning, deterioration, nutrition and hydration, medicines safety, risk assessments and documentation.
The post holder is also expected to contribute to the ward/clinical area accreditation programme and provide advice and support to teams about quality & safety.
