An exciting fixed term (12 months) opportunity has arisen for a Qualified Nursing Associate to join the Hambleton and Richmondshire Older Adult Community Mental Health Team. The applicant must have completed the Nursing Associate foundation degree and be a registered Nurse Associate on the NMC register. This post is not offering the training to gain this qualification.

The successful applicant will work as part of the busy community team offering an identified pathway to service users and act as a keyworker for a defined caseload. You will support with both medicines management and psychological approaches in your delivery of interventions to meet individual's needs. You will work collaboratively with service users and their families in order to support with goal setting and meeting identified need.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010

You must have the Right to Work in the UK in order to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.

Main duties of the job

The successful applicant will be responsible for delivering high quality, patient centred, compassionate care under the direction of a multidisciplinary team with a focus on promoting health.

Following initial patient assessment completed by the Registered Nurse you will be responsible for implementing care plans, including providing information and advice to patients, clients and carers. In collaboration with the Registered Nurse evaluate and adjust programmes of care in partnership with patients, clients and carers and the multidisciplinary team.

You will promote health and provide and monitor care with all patients and carers. Delivering interventions to a high standard in line with the Trust governance framework.

The role requires you to respond to enquiries from relatives and carers, dealing with concerns and problem solve to improve the patients experience, escalating to the registered nurse as required.

Accurately undertake risk assessments, using contemporary assessment tools to improve safety and quality of care alongside the registered nurse.

Complete medication monitoring for patients who are prescribed antipsychotic medications and Lithium. This includes completing ECG's and Venepuncture and supporting the Band 5 registered nurse to maintaining these physical health monitoring requirements.

In addition, there will be the opportunity to support group interventions for our non-governed therapies.
