Job overview
This is not an administrative post and it is not a clinical nursing post. It is something distinctive: a specialist coordination role that requires someone with healthcare experience, strong communication skills, the ability to manage complex information, and a genuine commitment to improving the experience of people living with a life-limiting condition.
The role includes a specific function in our Group 3 pulmonary hypertension pathway, providing coordination for patients with PH secondary to lung disease who are referred to Sheffield for specialist opinion and ongoing support following assessment.
The role also supports the sotatercept initiation programme and the programme for transitioning patients from intravenous epoprostenol to an alternative intravenous agent over the coming 18 months.
There is also a defined welfare and benefits signposting function, ensuring patients know what support is available to them beyond the clinic.
Main duties of the job
The Pulmonary Hypertension Clinical Care Coordinator supports the PVDU Clinical Nurse Specialist team in delivering safe, coordinated and patient-centred care to people living with pulmonary hypertension across Sheffield and a large regional catchment area.
The post-holder will coordinate complex clinical pathways, manage patient communication, support service data and audit processes, and act as a skilled signposting resource for patients navigating welfare, benefits, and community support. The PVDU covers patients from across Yorkshire, the East Midlands, and beyond.
Many patients travel two to three hours to attend clinic and live with high symptom burden during basic daily activities.
This role requires emotional resilience, excellent communication skills, and the ability to manage sensitive and complex information with consistency and care.
