Band 4 Health Care Support Worker – Urology CNS Team We are excited to offer a fantastic opportunity for a motivated and dynamic Band 4 Health Care Support Worker to join our dedicated Urology Clinical Nurse Specialist (CNS) team.

In this rewarding role, you will play a key part in delivering patient-stratified follow-up care for individuals diagnosed with prostate cancer. Working both independently and collaboratively, you will support patients throughout their post-treatment journey, under the guidance and supervision of the CNS team. Key Responsibilities

Deliver structured follow-up care for patients following prostate cancer treatment Support patients in self-management, empowering them to take an active role in their care Coordinate and provide personalised care, ensuring individual patient needs are met Offer emotional and practical support, helping patients navigate life after diagnosis and treatment Act as a consistent point of contact, fostering trust and continuity of care

About You We are looking for someone who is:

Main duties of the job

  • Work as a Health/Social Care Assistant to assist with the coordination and management of prostate cancer patients undergoing treatment at Hampshire Hospitals Foundation Trust to support patients both on and off treatment  •    Provide information to patients related to treatment/follow-up and signpost to appropriate healthcare providers within remit of role and guidance. •    Provide telephone advice and triage where appropriate, working within defined protocols and guidelines. Document and communicate outcomes as per service specification. •    Provide level 1 psychological support within own knowledge boundaries following appropriate training. •    Perform Holistic Needs Assessments using designated screening tools as appropriate. •    To participate in collating patient surveys and develop action plans alongside the senior CNS. •    Support CNSs in developing and facilitating Health and Well Being Programmes. This will involve contributing and delivering education to patient and carers, including information around health promotion, disease prevention and supporting self-management of their rehabilitation or long term condition. •    Document all interactions both verbally and written with accuracy and in line with Trust policy and procedure. •    Demonstrate excellent communication skills both verbal and written. •

Education

Essential

  • NVQ level 3
  • GSCE level education

Desirable

  • NVQ level 4

Experience

Essential

  • Previous participation in patient care and planning
  • Relevant Health care assistant or social care worker experience at AFC 3 or equivalent
  • Experience of working with the public
  • Experience of working with patients with cancer

Desirable

  • Awareness of Macmillan cancer agenda
  • Knowledge of relevant cancer treatments and terminology
  • Experience of discussing patient concerns and needs

Communication

Essential

  • Ability to demonstrate effective communication skills both written and verbal/non verbal

  • Demonstrate understanding of communication skills both written and verbal and non verbal

  • Able to plan and organise own workload
