Cheshire and Wirral Partnership NHS Foundation Trust (CWP) in collaboration with our Primary Care Network Ellesmere Port One (PCN) are excited to be rolling out the mental health Additional Roles Reimbursement Scheme (ARRS).

The post Holder will specialise in the area of Dementia.

These roles have been created as part of the NHS Long-Term Plan and mental health community transformation to put clinicians with mental health experience directly within primary care networks.

The funding for these roles is an exciting partnership between CWP and the PCNs as they are funded jointly between the two.  The person will be directly employed by CWP but jointly managed and supported by CWP and the PCN in a collaborative way of working.

Main duties of the job

To assess, treat and provide support for adults living in Ellesmere Port One footprint, who are experiencing memory issues. Providing a bridge between primary care and specialist mental health providers. Practitioner will work as part of the PCN MDT with Integrated pathway for patients with access to specialist mental health support. To Identify suitable patients for case management from the practice dementia registers and various mental health registers, and who have memory issues. Support of newly diagnosed patients and carers – linking into services, practical support. Links with 3rd sector Follow up and monitor of diagnosed patients and carers. Appropriate prescribing and monitoring on memory meds, annual reviews. linking with practice nurses, pharmacists Support for changing circumstances of patients and carers eg carer stress, declining dementia, challenging behaviour. Appropriate prescribing and monitoring of memory meds and others. Link with pharmacists, GP, ANP Support for end-of-life care for dementia patients – link with community teams, Macmillan, practices, care homes, GP, ANP Support for practice staff, PCN & community, for education.
