Main duties of the job
As a Practice Nurse at IC24, your role encompasses a blend of clinical expertise, continuity of care, and prevention. You'll support patients in managing long-term conditions, offer immunisations, conduct health screenings, and provide minor treatments. You'll work in a multidisciplinary team to deliver high-quality, patient-centred care. Your responsibilities include diagnosing and developing management plans for conditions such as diabetes and cardiovascular disease, undertaking NHS Health Checks, and offering preventative health interventions. You will ensure accurate recording of patient data and work independently and collaboratively with healthcare professionals to provide coordinated care.

What youll focus on
Delivering high-quality nursing care
Youll provide assessment, treatment, screening, health education services and advice to patients, working in accordance with the NMC Code of Conduct and practice protocols and guidelines.
Managing long-term conditions
Youll diagnose, monitor and support the development of individual management plans for patients with conditions such as diabetes, respiratory disease and cardiovascular disease, working in partnership with patients and other healthcare professionals.
Prevention, screening and health promotion
Youll undertake NHS Health Checks, cervical screening, immunisations, travel health advice, smoking cessation support and wider preventative health interventions.
Clinical procedures and treatment delivery
Youll carry out wound care, ear care, phlebotomy, ECGs, blood pressure monitoring and other routine clinical procedures safely and in line with practice policy.
Supporting quality and clinical governance
Youll ensure accurate coding and recording of patient data to support QOF and Enhanced Services and contribute to high standards of clinical governance and best practice.
Working within a multidisciplinary team
Youll work both independently and in participation with GPs, HCAs, General Practice Assistants and wider clinical colleagues to deliver safe, effective and coordinated patient care.
Risk assessment and safeguarding
Youll recognise safeguarding concerns and wider risk issues (including domestic abuse, child protection and vulnerable adults), ensuring appropriate escalation in line with policy.
