Job summary
We are seeking a resilient, flexible, and adaptable Practice Nurse to join our highly skilled clinical and supportive admin team. This temporary position is only suitable for a qualified nurse with a minimum of one years General Practice experience. If you have the skills and competencies to provide Practice Nursing and chronic disease plus proactive care to our ever expanding Practice population, we want to hear from you. You will be a confident practitioner, comfortable working autonomously without direct supervision, and capable of making and reporting independent clinical decisions with the experience required to step into the role and thrive.
This role is not suitable for overseas or non Visa applicants
Main duties of the job
As a core member of our clinical team, your primary responsibilities will include:
Patient Care & Chronic Disease Management
- Provide routine nursing care in accordance with clinical evidence, local, and national guidance.
- Implement and evaluate individual treatment plans for chronic disease patients, maintaining accurate chronic disease registers.
- Identify, manage, and support patients at risk of developing long-term conditions to prevent adverse health effects.
- Run chronic disease clinics, referring patients to secondary or specialist care when required.
- Support patients with the safe use of prescribed or over-the-counter medicines within your scope of practice.
- Provide comprehensive travel medicine services.
- Request and process pathology services as necessary.
- Provide home visits and care home ward rounds to our housebound and frail patients
- Provide immunisation clinics for both adults and children including baby clinics
Clinical Leadership & Support
- Prioritise health issues and intervene appropriately, supporting the team during clinical emergencies.
- Develop, implement, and embed health promotion and well-being programmes, delivering opportunistic health promotion where appropriate.
- Maintain and support Infection Control and cold chain procedures within the Practice
- Support the clinical team with all safeguarding matters in accordance with local and national policies.
- Maintain exceptional and accurate clinical records.
