Practice Nurse

Auchtermuchty Health Centre

An exciting opportunity has arisen at Auchtermuchty Health Centre for a Practice Nurse with experience in chronic disease management.

We are seeking an individual skilled in the management of one or more long-term health conditions, which could include:

• Diabetes• Hypertension• Heart failure• COPD• Asthma

Hours and working patterns are flexible to suit both parties and pay will be commensurate with skill and experience.

Key Responsibilities:• Long-term management of patients with chronic disease through clinic reviews.• Reviewing patient health questionnaires and test results.• Identifying and managing patients who would benefit from optimisation of their health through education, lifestyle change and medication.• Ideal candidate will be responsible for some stock ordering and replenishing.• Core nursing skills of wound care, phlebotomy, ECG taking, injections, cervical screening are essential (though it is important to note that this is not a large part of the role as many of these core services are now provided by our CTAC colleagues)

About Us:• Semi-rural GMS practice with approx. 6000 patients, using EMIS and DOCMAN IT systems.• A friendly team that works with the common goal of providing high quality health care to our local population.• Purpose-built premises.

• We have plenty of free onsite parking.
