Practice Educator - Community Falls

Job summary

The Specialist Falls Service within Hampshire and Isle of Wight Healthcare has an exciting fixed-term opportunity for an enthusiastic and motivated healthcare professional who is passionate about falls prevention, education and improving patient outcomes. This role will play a pivotal part in delivering our Community Falls Transformation Programme, helping to develop a more consistent, equitable and sustainable approach to falls assessment and management across Hampshire and the Isle of Wight. Initially funded until March 2027, with the potential for extension subject to further funding, the post offers a unique opportunity to influence practice at scale, support workforce development and contribute to reducing avoidable harm for some of our most vulnerable patients.

As Falls Practice Educator, you will work closely with community nursing, therapy, urgent care and specialist teams to deliver quality education, develop competency and confidence in falls assessment and prevention, and champion evidence-based practice. You will support staff through training, clinical supervision and quality improvement initiatives, helping to embed a culture of continuous learning and excellence across community services. This is an ideal role for a clinician with a passion for education who wants to shape the future of falls prevention, improve equity of access and leave a lasting legacy across our newly formed Trust.

As this is a trust wide role, it is essential that the successful candidate is a car driver.

Main duties of the job

As Falls Practice Educator, you will deliver high-quality falls prevention education, training and workforce development programmes across community services. You will work with community nursing, therapy, urgent care and multidisciplinary teams to build confidence and competence in falls assessment, prevention and management, supporting the implementation of our Community Falls Transformation Programme.

With the wider specialist team you will co-design and deliver engaging learning opportunities, develop competency frameworks, support clinical supervision and provide specialist advice to staff. Through joint working, and mentoring, you will help embed evidence-based practice, improve consistency of care and support better outcomes for patients at risk of falls.

We're looking for a motivated and enthusiastic registered healthcare professional with experience in falls prevention and a passion for education, quality improvement and service development. You will be an excellent communicator with strong organisational skills, the ability to influence and inspire others, and ideally will have experience of delivering training to healthcare professionals. The successful candidate will be self-motivated, adaptable and able to build effective relationships across a wide range of teams and services.
