The opportunity has arisen to be part of our expanding multi-disciplinary frailty team and deliver care in our Proactive Frailty Care Planning Clinics across the six Integrated Neighbour Team locations in Buckinghamshire.
In our Proactive Frailty Care Planning clinics we conduct Comprehensive Geriatric Assessments (CGAs) on moderately frail patients to identify and solve problems before things go wrong.
The post holder would work as part of an MDT within the clinics but would autonomously assess the therapy needs of the patients and provide the necessary solutions to ensure patients can lead healthier, more independent lives.
Main duties of the job
The Physiotherapist will provide expert care and treatment to patients within agreed protocols and clinical guidelines, working as an autonomous practitioner within their own competence; undertaking frailty health assessments and providing appropriate advice on therapy requirements.

Experience
Essential
- Experience of at least 18 months in a junior rotational Physiotherapy post
- Previous experience in core physiotherapy specialties including acute inpatient care, respiratory, Musculoskeletal and neurology.
- Attendance at post graduate courses in related fields and demonstrates reflective learning.
- Experience of managing own caseload and prioritizing
- Ability to work and interact effectively within an interdisciplinary team and contribute to decision making.
- Establish and maintain CPD Portfolio
Desirable
- Knowledge of areas of influence of psychosocial and cultural factors on health/illness behaviours
- Participation in audit and research and development activities
Special Circumstances

- Able to be empathetic and handle difficult or emotional situations, using judgement to determine optimal intervention.
Skills, Abilities & Knowledge
Essential
- Able to apply a range and depth of knowledge to the specialist clinical area of post.
- Able to make sound clinical decisions based on knowledge/experience and utilize advanced clinical reasoning skills.
- Able to carry out a range of assessments and procedures in order to make clinical assessments and diagnosis.
- Able to carry out manual therapy techniques and therapeutic handling, requiring skilful and strenuous physical effort during the day.
- Clear, concise verbal and written communication to ensure liaison with multi-disciplinary team, patients and carers to ensure records and reports are accurate, legal and in legible manner
- Ability to prioritise tasks and functions against well-reasoned standards.
Education, Qualification & Training
Essential
- Current registration with Health Professions Council
- Diploma /Degree in Physiotherapy
- Evidence of CPD in the form of a detailed personal development portfolio
Desirable
- MCSP Membership
- Specific formal courses appropriate to post specialty.
