Job summary
We are looking for a motivated & experienced Physician Associate to become an integral member of our Chronic Disease Review Team.
The successful candidate will review patients with a range of long term conditions, including asthma, COPD, coronary heart disease, hypertension, heart failure, and diabetes. Sound knowledge of these conditions and experience in their management is essential.
The ideal candidate will have excellent clinical assessment skills, strong communication and organisational abilities, and experience working within primary care or a similar setting. They will be proactive, able to work independently within their scope of practice, and committed to delivering high standards of care for patients with long-term conditions.
Main duties of the job
- Conduct comprehensive chronic disease reviews in line with current clinical guidelines.
- Make evidence-based recommendations for the initiation and titration of medications, with prescribing decisions reviewed and signed off by the GPs.
- Identify opportunities to optimise patient care and improve long-term condition outcomes.
- Support the effective management of Quality and Outcomes Framework requirements and local enhanced services.
- Work closely with the administrative team to ensure robust recall systems are in place and operating effectively, so that patients are reviewed in a timely manner.

The surgeries are located centrally and easy to get to from the M1 and surrounding areas.
Our main site at Worsbrough is a purpose built 'LIFT' building and our branch surgery at Gold Street is a purpose built privately owned surgery.

Job responsibilities
JOB SUMMARY
To work within a multi disciplinary practice team ensuring the provision of high standards of quality to the practice population. Undertake training and development as required within the role.
PRINCIPAL RESPONSIBILITIES
- To be responsible for the provision of the following clinics, as needed (after undertaking any necessary training/updates)
Respiratory & Cardiovascular Clinic
Flu clinic
Diabetic Clinic
- To ensure all staff members understand the format for accepting appointments.
- Management of emergencies e.g. anaphylactic shock, cardiac arrest and first aid.
- To liaise with colleagues to ensure there is appropriate cover for all services provided by the team.ADMINISTRATION 5. To attend meetings as required, contribute to discussions and ensure that any follow up action is implemented. 6. To attend clinical meetings , contributing to the agenda and discussions as appropriate 7. To attend annual appraisal. 8. Work in line with practice policies and procedures to maintain standards and working practices. 9. To ensure the maintenance of appropriate medical records, schedules and item of service claims.
DISEASE MANAGEMENT
- The post holder will be expected to be responsible for the following:Maintenance of chronic disease management & ensuring QuOF achievements
HEALTH PROMOTIONAL ACTIVITIES
- To give appropriate advice to the practice population promoting healthy lifestyles.
- To ensure that Health Promotional Literature is available in the Practice.GENERAL DUTIES
- Liaise with Executive GP/Business Manager regarding the maintenance and supply of equipment and medical sundries.
- Vaccination and immunisation of patients as per practice protocol
- Safe disposal of contaminated waste/sharps
- Management of emergency anaphylactic shock, cardiac arrest, first aid
- Maintenance of appropriate medical records and schedules
- Perform ECG
- New patient medical checks
- Family Planning adviceOther duties as necessary, but working in accordance with the UKCC Code of Conduct, Standards for Records and Record Keeping, Exercising Accountability, Standards for the Administration of Medicines and Guidelines for Professional Practice.PROFESSIONAL RESPONSIBILITYThe post holder will have appropriate supervision and be expected to use their own initiative in the role of the Physician Associate.The post holder will be required to undertake training, statements of competence or gain the equivalent level of skill acquired through experience in order to maintain professional development.BROADER RESPONSIBILITIESWhen necessary to act as a representative of the practice and to carry out the above duties in a responsible and respectable fashion at all times, particularly as the way the practice is managed may be occasionally taken to reflect the standard of care provided.Driving licence and car ownerAny other duties appropriate to the grading of the post.This job description will be amended in the light of developments within the practice; however, any changes will be discussed with the post holder.
