Job summary
Main purpose of the post-
To improve care and health outcomes for patients with improved access to care in general practice. The post holder will support patients to self-manage their well-being and long term conditions, through optimising medicines and provision of patient facing clinical pharmacy services.
The post holder will also mentor, coach and provide professional peer support to enable effective clinical supervision for less senior clinical pharmacists.
Scope and Range:
The post holder will:
Act as a credible and independent source of expert pharmaceutical advice to promote high quality evidence based cost-effective prescribing within safe management systems in primary care
Manage patients with more complex long term conditions and establish on going professional relationships with those patients for whom they are responsible
Contribute to the development of, and be responsible for leading, delivering and implementing the prescribing and medicines management strategy and systems throughout the practice.
Undertake clinical audit, review, monitoring and instigating change in primary care prescribing.
Work in collaboration with team members from primary, secondary and social care, and the patient to develop bespoke pharmaceutical care plans for individual patients

a- Patient care:Act as a credible and independent source of highly specialised, up to date, pharmaceutical advice to promote high quality evidence based cost-effective prescribing to GPs and the public (limited to the practice area). This advice involves obtaining, interpreting and evaluating medical research and other technical information in order to promote the clinical and cost-effective use of medicines.
Advise prescribers in order to optimise patients medicines to maximise treatment outcomes and minimise medicines-related risk and waste, through medication review and clinical audit within agreed protocols.
Special Requirements of the post
There is frequent:
Requirement to read and analyse patient records, make recommendations and reports to GPs and implement agreed changes.
Requirement to analyse and adjust patients medication and treatment regimens.
Requirement for the analysis of facts requiring decisions that are not always straightforward.
Requirement for the absorption, evaluation and dissemination of information to appropriate parties in a manner they can understand.

Job responsibilities

medicines administration issues e.g. poor compliance
patient concomitant medical problems
social issues
Organise and action drug changes in the practice for quality and cost reasons arising from work, communicating changes in medicines to patients either by letter, telephone, or in person and documenting changes made.
Manage patients with more complex long term conditions in patient facing clinics and, if required, by undertaking domiciliary visits
Undertake medication review for patients with complex long term conditions both from records and in patient facing clinics and, if required, by undertaking domiciliary visits
It is expected where necessary and appropriate that the post holder will work as an independent non-medical prescriber.
