Are you a motivated GP looking to develop your career within an innovative Primary Care Network (PCN) setting? If so, we would love to hear from you.
We are seeking a dynamic salaried GP to work across two of the PCN's GP Practices as part of our multi-disciplinary team delivering proactive, holistic care for patients with complex needs and long-term conditions. The position is for six sessions each week looking after patients from our Beauchamp House and Baddow Village Surgeries.
You will be a key player in our growing team, collaborating closely with multidisciplinary colleagues including GPs, nurses, pharmacists, paramedics, a digital and transformation manager, social prescribers, general practice assistants and care coordinators.

Main duties of the job
Fully qualified GP
Deliver consultations via face to face, telephone and online
Diagnose and manage acute and chronic conditions
Prescribe medications and review treatments
Manage long-term conditions, eg Diabetes, COPD and hypertension
Undertake home visits where appropriate
Undertake referrals to secondary care, where required
Maintain accurate clinical records
Work collaboratively with the multi-disciplinary team
Participate in clinical meetings
Support safeguarding responsibilities
Contribute to QOF and quality improvement initiatives
Adhere to NHS and CQC standards
Confident using data-driven approaches to guide clinical decision-making
Excellent communicator with a team-oriented mindset
About us
The four surgeries which make up Chelmsford City Health PCN Limited are separate practices that formed an network in order to benefit from shared resources and to be able to offer healthcare services at scale to its population of 43,000+ patients. Two of the practices are dispensing practices and all four are training practices.
You will be based at two of these practices, Baddow Village and Beauchamp House Surgeries, and this will be an excellent opportunity to work alongside experienced colleagues as part of a friendly, progressive PCN committed to improving community health and to provide collaborative and patient-centred care.

The position is for six sessions per week looking after patients from our Beauchamp House and Baddow Village Surgeries.

You will be a key player in a growing team, collaborating closely with multidisciplinary colleagues including GPs, nurses, pharmacists, paramedics, a digital and transformation manager, social prescribers, general practice assistants and care coordinators.
