This is a key facilitation and support role at the heart of the Trust’s Patient Safety and Clinical Risk work. You will play a vital part in ensuring that patient safety incidents and learning processes are well‑managed, robust and translated into meaningful improvement, communicating learning identified in a relatable and accessible format across the organisation.

Main duties of the job

We are seeking a highly organised and proactive facilitator to provide comprehensive support and expertise across the Patient Safety and Clinical risk portfolios.

The postholder will ensure that incidents and near misses are reported and investigated in accordance with the Patient Safety Incident Response Framework (PSIRF), reported to the Care Quality Commission (CQC) where appropriate and that learning is shared across the organisation to drive sustained improvement in patient care. They will facilitate the management and quality assurance of the Trust Risk Registers, giving advice to risk owners, assessors and senior colleagues and support with preparation of reports for Trust Executives and Committees.

• Review incident reports, sharing and escalating any incidents of concern through appropriate Trust processes, including Incident Safety Panel.
• Provide expert advice and information on incident reporting, review & management as well as on wider patient safety matters using a range of communication methods.
• Ensure that Statutory Notifications are made to the Care Quality
• Commission in a timely manner, providing support and guidance to service managers where required.
• Extract learning identified from Learning Response Tools to capture themes and actions.
• Produce a monthly Patient Safety bulletin for clinical directorates and other relevant teams to support organisational learning.
• Maintain the Patient Safety SharePoint site ensuring key information is up to date and accessible to colleagues across the organisation.
• Work with the Patient Safety and Mortality Co-ordinator in maintaining efficient systems and processes to ensure good governance and enable the effective and accurate collection, collation and reporting of routine data and information, producing statistics when required.
• Work with the Patient Safety and Mortality Co-ordinator and clinical directorates to ensure that all Patient Safety Incident Investigations (PSII) are reported to the Integrated Care Board (ICB) and Care Quality Commission (CQC) within appropriate timescales.
• Work with the Patient Safety and Mortality Co-ordinator and Senior Managers to monitor the implementation of Safety Improvement Plans (SIPs).
• Support the Patient Safety and Clinical Risk Manager in the production of reports to the Trust Board, ICB and other internal groups and committees on patient safety and clinical risk.
