Currently, Cornwall has one of the highest rates of hospital stays during the last 90 days of life, a clear signal that change is needed. We are creating a more patient-centred, community-based approach that brings compassionate care closer to home in the last year of their life or nearing the end of life.

As part of our growing service, we are:

  • Reducing unnecessary hospital admissions
  • Delivering personalised care plans to support home-based care

In collaboration with the Macmillan Social Investment Fund and local partners, we're expanding services including:

  • A 24/7 Rapid Community Response and Care Service
  • A dedicated End of Life Care phone line for public and professionals
  • Strengthened voluntary sector partnerships and our Hospital@Home virtual service

We have opportunities available across various localities within the county, offering flexibility in base location depending on service need and candidate preference with a requirement to cover Cornwall and the Isles of Scillys.

Main duties of the job

This job is currently only open to people who already work for the NHS in Cornwall or the Isles of Scilly.

To undertake a clinical lead role in Palliative Care and Cancer within the Community Rehabilitation  Team.  To provide a high standard physiotherapy service, performing advanced assessments of patients including those with highly complex presentations and formulating a diagnosis and treatment plan which is carried out as an autonomous practitioner.

Skills and Aptitude

Essential

  • Able to work in highly stressful situations including the ability to manage distressing and highly emotive situations successfully (eg. Working with people recently diagnosed with life changing conditions).

Education and Qualifications

Essential

  • Current professional registration – HCPC, CSP
  • Professional qualification – Physiotherapy
  • Evidence of further formal study in Advanced Clinical Practice eg. Post graduate qualifications(Post Grad Certificate/Diploma/MSc/Non-Medical Prescribing)
  • Perform advanced assessment of patients including those with unpredictable, varied and highly complex presentations.
