Are you a passionate Occupational Therapist or Physiotherapist looking for a rewarding role within a dynamic and evolving Urgent Care Service? Join our multidisciplinary team delivering short-term care, rehabilitation and therapy interventions that help people remain safely at home, avoid unnecessary hospital admissions and support timely hospital discharge.
Working across community settings, you will assess, plan, implement and evaluate holistic care for patients with complex needs, promoting independence through rehabilitation and enablement. You will manage your own caseload, undertake triage and urgent assessments, identify and respond to clinical deterioration, prescribe appropriate treatment interventions, coordinate equipment provision and contribute to complex manual handling plans. You will also play a key role in supporting innovative models of care, including Virtual Wards, enabling patients to receive hospital-level care in their own homes through remote monitoring and multidisciplinary support.
Main duties of the job
We are seeking an experienced, motivated and adaptable HCPC-registered Occupational Therapist or Physiotherapist who thrives in a fast-paced environment and is committed to delivering high-quality patient-centred care. You will be confident working autonomously while collaborating effectively within a multidisciplinary team, providing specialist advice and supporting colleagues with complex assessments and clinical decision-making.
The successful candidate will have excellent communication and problem-solving skills, experience managing patients with complex health needs, and a passion for supporting people to achieve maximum independence. You will contribute to service development, quality improvement initiatives and the delivery of business and operational objectives, while supporting the Clinical Lead in the day-to-day running of the team. This is an exciting opportunity to be part of a service that operates 24 hours a day, 365 days a year, providing urgent care, rehabilitation, admission avoidance, discharge support and end-of-life care in the community. If you're looking for a role where you can make a real difference every day, we'd love to hear from you.
