An exciting opportunity has arisen for a Occupational Therapist to be part of the Home First service in Staffordshire.

This is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs within Midlands Partnership University NHS Foundation Trust (MPFT).

The Home First Team prides themselves on the delivery of quality care to community patients, many with complex needs.

The team also work to facilitate safe hospital discharges for patients leaving acute care and to offer them nursing support in their own home environment.

This is a dynamic post which will be evaluated and adjusted to reflect the needs of the service, local population and feedback from users and staff. Your feedback and input will be essential to the development and shaping of the role.

The North Home First covers the whole of North Staffordshire and Stoke-on-Trent providing clinical support for patients in the community to either avoid potential hospital admissions or to support people returning home through the Discharge to Access pathway following a period of acute care.

Main duties of the job

The post holder will be part of the Allied Health Professions (AHP) service providing Occupational Therapy and will work in a variety of settings.

This is a dynamic post that requires the post holder to work closely with uni/multi-disciplinary teams/agencies in addressing a range of care/support issues across the spectrum of conditions, preventing unnecessary hospital admissions and improving the health and wellbeing of the population served. The nature of the service will be determined by local service specifications

  • Be able to travel

Experience

Essential

  • Relevant experience to the role including assessment and interventions for a wide range of disabilities.
  • Ability to manage time and caseload effectively
  • Supporting service users, families and carers in dealing with the impact of disability and limiting life conditions
  • Evaluate the outcomes of the individual's rehabilitation
  • Discharge and transfer individuals where appropriate
  • Support individuals to manage their condition/disability Identify and evaluate opportunities for innovation and improvement

Desirable

  • Knowledge of relevant NSF's and NICE guidance. Understanding of wider NHS agenda and its application to the local context.
  • Continuing healthcare assessments
  • An understanding of team dynamics

Qualifications

Essential

  • Degree in Occupational Therapy or equivalent.
  • Registration with HCPC/NMC
  • Fieldwork educators accreditation

Desirable

  • Modules at Masters level
  • Evidence of further clinical study
  • APPLE accreditation
  • Leadership experience/qualification
  • Membership of BAOT / COT
