We’re looking for a motivated and enthusiastic Occupational Therapist to join our Intermediate Care service, working across community and hospital pathways to support people to regain independence and return home safely.
This is a great opportunity for a newly qualified or experienced Band 5 Occupational Therapist to develop their skills within a supportive, forward-thinking team. You’ll manage your own caseload, completing assessments, delivering interventions, and working closely with colleagues across health and social care to achieve the best outcomes for the people we support.
You’ll be supported to grow as an autonomous practitioner, with access to regular supervision, peer support, and development opportunities tailored to your experience. For those earlier in their career, this offers a strong foundation to build confidence and clinical skills; for more experienced Band 5s, there are opportunities to further develop clinical expertise and contribute to service development and support improvements in pathways.

Main duties of the job
As a Band 5 Occupational Therapist within the Intermediate Care service, you will manage a varied clinical caseload, supporting people to regain independence and achieve safe and timely discharge from hospital or community pathways.
In this role, you will:
- Manage your own caseload
- Carry out holistic assessments and identify needs and risks
- Plan and deliver person-centred interventions
- Work closely with the wider team to support discharge and flow
- Prioritise work and respond to changing demand
- Keep clear, accurate clinical records
- Delegate appropriately to therapy assistants and support their development

Qualifications
Essential
- BSc/MSc or Diploma in Occupational Therapy
- Evidence of undergraduate CPD in the form of a detailed personal development portfolio.
- Evidence of relevant post graduate training.

- Clinical Educators Accreditation

- Demonstrates good written communication skills
- Demonstrates good verbal communication skills
- Demonstrates flexibility to meet the demands of the service
- Demonstrates good organisational and time management skills
- Shows evidence of clinical reasoning skills demonstrating knowledge of medical, surgical, neurological, mental health conditions and discharge planning skills.
- Shows good IT skills with understanding of computers and their applications
- Demonstrates organisational skills including, time management prioritisation and the ability to complete a task.
- Physically fit and able to comply with the Trusts moving and handling guidelines subject to reasonable adjustments under the terms of the Equality Act 2010.

- Ability to travel to different geographical locations across the Trust footprint in line with service needs.
- Full UK driving license with willingness to use own car for work purposes.

Knowledge, Training & Experience
Essential
- Broad range of experience at undergraduate level including managing medical, surgical, neurological and mental health conditions.
- Knowledge of anatomy, physiology and psychology to support relevant clinical skills and effective clinical reasoning.
- Experience of working within a team and independently with support.
- Experience of liaison with other professionals and partnership working.
- Demonstrates an awareness and understanding of supporting equality and valuing diversity within the role. Evidence and understanding of CPD
- Uses examples to demonstrate a clear knowledge and understanding of the NHS.
- Knowledge of areas of clinical governance including quality, audit and risk management.
- Ability to work autonomously within code of practice with peer support.
- Able to present information in a clear and logical manner.
