Job overview

We are seeking a motivated Occupational Therapist to join our Locality Therapy Team. The successful individual will be based at  Ludlow Hospital, working on the Community Hospital ward and as part of the Community Team visiting patients in their own homes. They will join an enthusiastic and supportive multi-disciplinary team in providing high quality care, rehabilitation and discharge planning.

The successful individual must have an interest and experience in patient rehabilitation with in the Community setting and be able to work as an autonomous practitioner.  The post holders’ main responsibilities will be providing Occupational Therapy assessment and rehabilitation to patients who have a wide range of conditions and complex needs.

We have a strong commitment to clinical supervision and Continuing Professional Development (CPD), offering a supportive and proactive environment with opportunities for supervision, training and peer support, both locally and County wide.  This post will be involved in supervision of junior staff, rehabilitation technicians and students.

Main duties of the job

  • To provide a high quality seamless Occupational Therapy service from Acute or Community Hospital to Community, responsive to the needs of the local population as part of a cohesive multidisciplinary team.
  • To work as a key member of the multidisciplinary team (MDT) that is responsive to the needs of the local population, working flexibly to respond to the changing needs of the patient.
  • To provide a key role in facilitating the seamless transition from community hospital to patient’s own home and transferring the care of the patient to other appropriate services to maximise the patient's independence and quality of life.
  • To provide supervision, facilitate the personal and professional development of junior members of staff, enhancing their skills and knowledge and where appropriate line management.
