Job summary
We are looking for a newly qualified/experienced occupational therapist who is keen to deliver high quality care in the community setting. You will be part of an integrated team of therapists, therapy assistants, and nurses who provide essential knowledge and skills to support patient discharges, prevent unnecessary hospital admissions and keep people safe in their own homes.
Your caseload will be varied with an opportunity to see patients with a variety of presenting conditions allowing you to develop and improve existing problem-solving skills.
Worcester City have two Neighbourhood teams. Cityside are based in Elbury Moor medical centre and Riverside are based in St Johns House surgery. We have a team of creative health care professionals focused on compassionate care. We work closely with GPs, developing relationships with other primary care services, utilising skills of our MDT colleagues to manage complex patients ensuring services are responsive and patient centred.
You will have opportunities for further training and development within the role. You will be joining an enthusiastic, supportive, and friendly team where you will receive support and development through in-service training, regular appraisals, a personal development plan and clinical supervision.

Main duties of the job
A day shift is 7.5hrs between the hours of 07:30 18.30, Monday to Friday. Your typical day will include coordinating your caseload, triaging referrals, allocating workload and prioritising urgent response assessments alongside colleagues.
The role will involve you:
Undertaking occupational therapy assessments for a designated caseload, addressing occupational performance and skill deficits, enabling the client in areas of self-maintenance, productivity and leisure.
Assessing clients: in relation to their equipment and specialist equipment needs / for minor adaptations e.g. rails, half steps.
Working with clients to identify Occupational Therapy goals as part of the overall care plan.
Assist in any other duties that may be required to ensure the smooth running and efficiency of the service.
Monitoring and evaluating treatment in order to measure progress and ensure effectiveness of the intervention.
Maintaining contemporaneous clinical notes to professional minimum standards of the College of Occupational Therapy and in accordance with Trust guidelines.

Essential
- Some knowledge of current best practice in Community Occupational Therapy.
- Understanding of professional ethics and their application in practice.
- Basic knowledge of the principles of clinical governance

- Previous relevant NHS experience.

- Use of functional assessment and activity analysis
