<p>As a Band 6 Occupational Therapist, you will join our Early <span class="single_desc_int_gb_word">Supported Discharge Service based on Sapphire Ward at Gravesham Community Hospital</span>. This is a rewarding opportunity to <span class="single_desc_int_gb_word">support patients recovering from stroke and neurological conditions as they transition from hospital to home. You will play a key role in delivering high quality, patient centered rehabilitation, helping people regain independence and confidence in their daily lives. This is a varied role that combines inpatient work with community visits, giving you a unique opportunity to follow patients through their recovery journey</span>.
