Join the Merton Domiciliary Therapy team and play a pivotal role in delivering high-quality rehabilitation services to the local community. As an Occupational Therapist, you will be a key member of a multidisciplinary team (MDT), working collaboratively to support the rehabilitation of adults with frailty and complex health conditions. Your expertise will help patients optimise their independence, maintain their wellbeing and prevent unnecessary hospital admissions or premature moves to care homes.

In this role, you will assess and provide tailored treatment plans to patients in their homes, offering specialist support to enhance mobility and functional recovery. You will also contribute to the development of rehabilitation strategies and educate and supervise junior staff and students, ensuring the delivery of exceptional care. With a strong emphasis on fostering a culture of respect and support, you will help drive service improvements, providing clinical leadership and contributing to continuous professional development.

Main duties of the job

  • Deliver high-quality, person-centred rehabilitation to patients in their homes, care homes, and community settings as part of the MDT.
  • Assess and provide tailored OT interventions for patients with complex physical, cognitive and functional needs.
  • Manage a caseload of patients, working autonomously and collaborating with MDT colleagues to ensure coordinated care.
  • Promote independence through rehabilitation, mobility support, equipment provision and environmental adaptations.
  • Supervise and mentor junior staff, students and rehabilitation support workers, fostering a supportive learning environment.
  • Lead quality improvement initiatives and contribute to service development, audits and clinical pathway improvements.
  • Liaise with statutory, voluntary and community services to support integrated care delivery.
  • Maintain accurate clinical records and participate in governance activities to ensure high standards of care.
  • Support the professional development of the team through training and continuous learning.
