Job summary

We have an exciting opportunity for a Band 5 occupational therapist who is dynamic and passionate about stroke rehabilitation to join our supportive and caring team.

The Community Stroke Rehabilitation Team offers specialist multi-disciplinary rehabilitation to adults who have had a stroke in a community setting.

You will work closely with other professionals such as physiotherapists, speech and language therapists, psychologists, dietitians, nurses, therapy assistant practitioners and rehabilitation assistants.

The service provides goal focused, time limited rehabilitation interventions in patients home environments and in other settings as appropriate.

The stroke rehab team work closely alongside colleagues from Leeds Teaching Hospital Trust, as well as third sector organisations such as the Stroke Association, to support patients along the whole stroke pathway, from hospital to home.

This is an exciting time to join the team, as we embark on implementing a new intensive rehabilitation pathway to support earlier discharge from hospital alongside our current offer.

The ideal candidate will have experience experience of working with people who have had a stroke or other neurological conditions, MDT working and community working.

Main duties of the job

The postholder will be part of designing and implementing a new more intensive pathway which will allow patient to be discharged from hospital sooner.

The post holder will support unregistered staff within the team as well as providing advise and guidance to staff from the MDT. They will have strong clinical skills backed up with evidenced based practice. The ideal candidate will be a highly motivated individual, who is passionate about helping patients to improve their quality of life and who can collaborate well with the team.
