Join our Brent Urgent Community Response (UCR) Team as an Occupational Therapist and make a meaningful difference to people’s lives. You will deliver expert, home-based care that supports individuals to remain independent, recover safely and avoid unnecessary hospital admission.

Main duties of the job
The post holder will provide high-quality occupational therapy assessment, intervention and rehabilitation to patients across a range of community settings, including care homes, residential units and individuals’ own homes.
You will work closely within a multidisciplinary team to support admission avoidance, facilitate early hospital discharge and promote independence and recovery.
Key responsibilities include:
- Undertaking holistic assessments and delivering evidence-based interventions tailored to individual needs.
- Prescribing, ordering and providing education on the safe use of equipment and adaptations.
- Contributing to effective care planning and risk management to support safe care at home.
- Supervising, mentoring and supporting the development of junior staff and students.
- Building strong, collaborative relationships with patients, families, carers, colleagues and partner agencies.
You will be expected to work flexibly and contribute to the delivery of a responsive, patient-centred service that meets the needs of the local population.

Experience
Essential
- Relevant work experience in a variety of clinical Settings.
- Evidence of up-to-date CPD
- Experience working as part of an MDT.
- Use of functional and cognitive assessments.
Desirable
- Evidence of previous employment as an Occupational therapist at Band 5 grade
- Experience working in the Care of Elderly and with Acutely unwell patients
- Experience in Environmental Assessment and Minor Adaptations
- Experience as a community equipment store Trusted Assessor

- Knowledge of current best practices in Occupational Therapy and demonstrate clinical reasoning, applying theory to practice.
- Demonstrate an ability to work single-handedly with individuals or in a team.
- Understanding of professional ethics and their application in practice
- Adherence to national, professional, and work-related policy
- Health, safety, and risk awareness
- Demonstrate an understanding of clinical governance and its implications for the profession.
- Basic knowledge of health legislation and current practice, risk assessment and management
- Good organisational skills, ability to prioritise and manage time/caseload effectively
- Effective written and oral communication skills. Ability to listen and present written and verbal information concisely and logically.
- Good IT skills (word processing and internet)
- Demonstrate commitment to lifelong learning, with the ability to reflect and critically appraise one's performance.
- Commitment to client-centred, non-discriminatory practice
- Understanding of team dynamics and good interpersonal skills

- Awareness of the legal responsibilities of the profession.

- Diploma/Degree in Occupational Therapy
- Registration with the Health and Care Professions Council
