NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non-clinical professions and roles. The role:
The Long-term Conditions service is an AHP-led, digital-first self-management service. Patients referred to the team have been diagnosed with a long-term health condition which has impacted their quality of life and ability to function, which may include difficulties with work. Common conditions could include ME/CFS, fibromyalgia, post-viral fatigue and Long Covid, etc.
Care is provided via a 6-week virtual self-management group, specialised workshops and targeted 1-1 interventions (often via near me). This is a hybrid role; therefore, work will be undertaken remotely from home, through community home visits, and by attending one of our remote working bases (Rowanpark or Belmont) or local health centres, as required. The Long-term Health Conditions Service works across 6HSCPs within NHSGGC boundaries. A driving licence and access to transport to support visits across this geography are needed.
The department provides:
All staff within the Services must comply with NHSGGC Policies; in addition to this, qualified staff will comply with Royal College of Occupational Therapists (RCOT) Professional Standards and Code of Conduct and HCPC Standards of Proficiency.
Candidate:
Successful candidates will have an occupational therapy degree (or equivalent) and be HCPC registered. Able to evidence community experience with a range of conditions. As this is a board-wide role, a driving licence and access to a car are essential.
The post holder is a member of the team within the AHP Long Term Conditions Service and will provide Occupational Therapy interventions within the team.
Has responsibility for the daily functions of the Occupational Therapy service within the AHP Long Term Conditions Service
Has a key role in the education and development of Occupational Therapy and other staff within the AHP Long Term Conditions Service
Active involvement in Research and Development within the AHP Long Term Conditions Service
Provides a specialist resource to other health care staff/patients/carers / other agencies

Key responsibilities:
Delivers specialist interventions within a brief interventions model using a digital first approach, using the principles of activity analysis and a wide range of treatment techniques, i.e. neurodevelopmental, therapeutic handling techniques, functional rehabilitation, psychological and emotional wellbeing adaptation, equipment provision, vocational rehabilitation, education and health improvement
Works as a key member of a multi-professional team to ensure effective communication and delivery of care
Prescribes and coordinates intervention to be carried out by others, such as support workers, including training to carry out therapeutic programmes as appropriate
Attends and reports in clinical meetings and case conferences
Provides specialist advice, when required, on the management of patients within the AHP Long Term Conditions Service, e.g. strategies for coping with perceptual/cognitive deficits
