Are you a qualified Nursing Associate looking to develop your skills within a supportive community team? This is an exciting time to join our Community Nursing Service in Hertfordshire.

We are seeking a qualified Band 4 Nursing Associate to work 30 hours per week, delivering high-quality care to people in their own homes and local communities.

We are developing new community pathways to support more care in people’s homes. Through the CLCH Academy, you will have access to a range of learning and development opportunities. You will be supported by Professional Development Nurses and your Team Leader to build the knowledge and competencies needed for your role. We have a strong track record of supporting Registered Nursing Associates to complete the Registered Nurse Degree Apprenticeship Programme.

You will work as part of a multidisciplinary team, providing safe, compassionate and effective care. Strong communication skills, adaptability and a commitment to treating patients with dignity and respect are essential.

As this is a home visiting service, you will need to travel between locations. A driving licence and access to a vehicle for work purposes are essential, unless reasonable adjustments apply under the Equality Act 2010.

Main duties of the job

Key responsibilities of this role include:

• Providing high-quality, evidence-based nursing care for an allocated group of patients.
• Acting as an accountable nursing professional, practising in line with the Nursing and Midwifery Council (NMC) Standards of Proficiency.
• Recognising and working within the scope of your competence, seeking support and guidance when required.
• Working collaboratively as part of a multidisciplinary team to deliver effective, patient-centred care and make efficient use of resources.
• Contributing to the delivery of safe, high-quality care while supporting the wider nursing team.
• Undertaking a range of clinical procedures and interventions appropriate to the role.

In this rewarding role, you will work closely with patients, community nursing colleagues, GP practices, and wider multidisciplinary teams.

The role includes participation in a weekend rota, typically involving three weekend days within a four-week roster cycle.

Applicants must hold a relevant Nursing Associate qualification. Experience in a community setting would be beneficial but is not essential.
