The Learning Disability and Autism Liaison Team are recruiting a Band 4 Nursing Associate to work across east and west Suffolk as required, to support the registered practitioner staff in delivering the service.

You will be pivotal in ensuring people with a learning disability have their physical and mental health needs met in primary care services in a timely and appropriate manner. The service is provided in GP practices and across various other community settings, including outreach into people's homes and operates from bases in Ipswich and Bury St Edmunds. It operates 9-5pm, Monday to Friday and as part of NSFT's Learning Disability & Autism Specialist Pathway has close links with the community learning disability teams and other teams within primary care services

Main duties of the job

Are you able to think and work creatively with individuals and their carers? You will be able to work independently, supporting the delivery of person-centred care plans with a focus on prevention of ill health through promotion of healthy lifestyles. Communication and engagement are key for people with learning disability, so these are key skills required along with a positive and creative approach. You will be able to demonstrate that you are able to apply your skills to a wide range of situations, working with a range of people with a learning disability in the community.

The team offers:

  • Consultation, advice, brief assessment and formulation to GPs on the needs of adults with learning disabilities who may also have associated physical health conditions, behavioural difficulties and/or mental illness. Advice and support to individuals and families about health needs. Support to develop accurate Learning Disability registers, attendance at annual health checks, and health action planning for people aged 14 and over with learning disabilities. Signposting to access the use of health and social care services and other organisations. Supporting access through reasonable adjustments for people with a learning disability and autistic spectrum conditions.

You must have a full and valid driving licence and have access to a vehicle due to the community element of the role and the large geographical area that the service covers. The West team is based in Bury St Edmunds and the East team is based in Ipswich.
