Job overview

An opportunity has arisen for a Band 6 Registered Nurse or Paramedic to join our team, working across an integrated Urgent Neighbourhood Health Service, including the Urgent Community Response and Hospital at Home services. This is on a fixed term contract for 12 months.

As part of a multidisciplinary team, you will deliver high-quality care to patients in their own homes and community settings. Using your clinical and diagnostic expertise, you will assess, manage and support patients with acute health concerns, complex health needs and long-term conditions.

You will work autonomously within your professional scope of practice while collaborating closely with colleagues to provide responsive, holistic care.

A key focus of the role is supporting admission avoidance and enabling timely, supported discharge from hospital where appropriate.

You will:

• Take responsibility for managing the caseload and the team, with the support of the Team Leader.
• Support locality-based planned care teams and partner services when acute or intensive nursing care is required.
• Actively contribute to service development initiatives. Working Hours:
• Work on a rotational shift pattern within Urgent Neighbourhood Health Services (service hours: 08:00 to 23:00).

Main duties of the job

Key duties within this role include:

• Provide day-to-day management of the service on a rotational basis in the absence of the Band 8a Clinical Lead.
• Take responsibility for designated areas of clinical practice and operational delivery.
• Prioritise and manage own workload, seeking guidance and support when required.
• To provide skilled, effective and evidence-based nursing care to patients in a variety of community settings.
• To assess referrals made to service, prioritise and respond according to priority, as appropriate both in terms of level and urgency. To respond within agreed timescales as appropriate.
• Utilise advanced clinical and diagnostic skills and expert knowledge to carry out thorough assessments of physical and psycho-social needs in order to develop a comprehensive short term care plan aiming to optimise health and wellbeing that will prevent a hospital attendance / admission.
• Provide direct advanced, autonomous clinical care to a range of patients presenting with differentiated diagnosis or RED flags, undiagnosed and/ or complex health needs and long-term conditions.
• Work within Clinical Guidelines, Policies, Procedures and Patient Group Directions (PGD’s) utilising expert judgment to work outside these frameworks where necessary.
