Are you an passionate nurse educator seeking a specialist community role with system impact? Join the Provider Improvement and Response Team (PIRT), improving outcomes for residents in care and nursing homes across Staffordshire and Stoke-on-Trent.

Secondment applications are welcome, subject to manager agreement. Alternatively, a fixed term contract can be offered.

About the role You will work autonomously within a multi-disciplinary team, supporting care home providers to improve safety, wellbeing and outcomes. You will undertake holistic assessments, deliver targeted interventions, build staff capability and contribute to admission avoidance.

You will also support system learning, quality improvement, and work with partners across Local Authority, primary care, acute and voluntary services.

Main duties of the job • Build strong relationships with care homes, partners, and multidisciplinary teams to support high-quality, coordinated care. • Support care homes to identify risks, address challenges, and improve resident outcomes. • Provide clinical advice, guidance, and practical support across areas including safety, falls prevention, infection control, and wellbeing. • Deliver education, training, and workforce development to improve confidence, capability, and quality of care. • Support early identification of deterioration and contribute to reducing avoidable hospital admissions. • Identify themes, risks, and learning from incidents, contributing to service improvement and quality initiatives. • Maintain accurate records and effective communication across teams and organisations. • Work autonomously with professionalism, sound judgement, and effective workload management. • Represent the team at partnership meetings across the care home sector and wider system.
