An exciting opportunity has arisen for a Nurse Associate to join the South East IDT  Team in the provision of  Planned Care to patients living in the community. We are a proactive and forward thinking team, who constantly strive for the delivery of excellent patient care and modern ways of working.

As a Nurse Associate, you will be supported to develop a broad range of clinical skills to enable you to provide evidence based care. You will receive additional training and clinical support specifically in regard to Continence Care & wound management .We are committed to clinical support, CPD & training.

There is a need for excellent communication skills and a dedication to multi agency working; you will need to work flexibly within a busy, demanding environment with the ability to develop new skills and working patterns.

Main duties of the job

  • Provide compassionate, safe and effective care and support to people in a range of care settings.
  • Ensure the privacy, dignity and safety of individuals is maintained at all times.
  • Monitor the condition and health needs of people within their care on a continual basis in partnership with the members of the MDT team families, and carers.
  • To complete nursing risk assessments using recognise risk assessment tools.
  • Contribute to the planning of care delivery in association with the Registered Nurse .
  • Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment and action.
  • Once passed as competent to administer medicines as directed.
  • Demonstrate the ability to recognise changing priorities seeking advice and guidance from the Registered Nurse or other registered care professionals as appropriate.
  • Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care.
  • To oversee the care delivered to their designated group of patients by Health Care Assistants. Trainee Nursing Associates and Nursing Students.
  • Recognise and report any situations, behaviours or errors that could result in poor care outcomes.
