Job summary

The Barnsley Primary Care Network would like to recruit Qualified Nurse Associates to complement our current cohort within the PCN and the clinical workforce as a whole. These roles will be based at assigned practices within a Neighbourhood of our Network, supported by a Practice Nurse for mentorship.

The role of the Nursing Associate is to work with the Registered Nurse and other members of the multi professional team, providing and monitoring care and contributing to the ongoing assessment, as agreed with the supervising clinician(s) and in line with the service pathways. They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi professional team, and will be involved in shaping and supporting patients to manage their clinical care needs and you will be central in ensuring they enjoy healthy fulfilling lives.

The role will be based in Goldthorpe Medical Centre and Full time.

Main duties of the job

The Nurse Associate plays a key role in shaping the delivery of nursing care for the future. We are looking to recruit motivated and enthusiastic individuals to support our nurses to deliver quality clinical care.

We expect that you will be NMC Registered, able to work independently and manage your own workload. You will need excellent communication skills, able to meet targets and liaise with patients, primary care staff and other partners.

The PCN is interested in supporting individuals with an interest in the Primary Care environemtn to develop their skills within this setting.

Job responsibilities

PRIMARY DUTIES AND AREAS OF RESPONSIBILITIES

The post holder will be responsible for administering immunisations, using behaviour change and under a PSD

Smoking Cessation clinics

Maintaining Infection Control Standards

Health Promotion advice to patients

Supporting the General Practice Nurse with QOF

Flu planning

Recall Child Health,

Maintaining Safety and Quality

Checking Equipment / Emergency Drugs oxygen etc.

Use behaviour change interventions to support health and wellness, (for example, NHS health checks)

Cervical Screening following the Cervical Screening Training

Providing and monitoring care

The post holder will they contribute to the ongoing assessment of care and be able to recognise when to refer to Senior Colleagues in practice.

Demonstrate an ability to undertake baseline physical observations and record results correctly. In addition, be able to identify anomalies that should be referred to the GPN / ANP or GP Medical staff.

The post holder will contribute to the ongoing assessment of care, undertaking baseline observations of Blood pressure, Pulse, Respirations, Weight, Height, Elimination pattern, Eating pattern, Sleep pattern, Glucose monitoring

Discuss and plan care under the supervision of registered staff, where applicable escalating concerns or complex cases to the GPN/ANP or GP

Demonstrate evidenced based knowledge in the main physical health problems that adult patients may present with i.e., High blood pressure, Diabetes, Obesity, Epilepsy, Respiratory Conditions, Asthma COPD, Chronic Kidney Disease, Cardiovascular Disease, Poor sleep, Constipation, Foot care and Lack of personal hygiene.

Demonstrate evidenced based knowledge in Mental problems/ Substance Misuse, which may affect physical health.

Manage situations with patients this may arise from anxiety / emotions relating to poor mental health and or direct them to GPN / ANP/ GP as appropriate.

Articulate an understanding of MDT in and staff roles/ Mental Health, community Nursing 0-19 services etc.

Assess and manage risk associated with the working environment ensuring appropriate BHF policies and procedures are followed accordingly.

Demonstrate an ability to use a range of therapeutic techniques when dealing with patients with challenging and unpredictable needs.

Understands the concept of verbal and non-verbal communication and be able use the skills at the desired level.

Will be able to discuss and advise patients, with chronic conditions and carers or relatives.

Will communicate with the patient about care planning and risk assess report to GPN as appropriate.

Communicate complex and sensitive information verbally and in writing.

Confidently and accurately convene patient information in the role of an associate nurse in meetings

Understands risk behaviours and risk management.

To record Incidents in line with local policy procedures and standards.

To communicate risk issues as they arise to ensure safe care.

The post holder will engage in positive relationships with staff/ patients/carers/families and others based on respect and within appropriate boundaries.

Act in a professional capacity at all times.

Actively contribute to staff Governance meetings.

The nursing associate may wish to lead responsibility in a chronic disease area for example (once more expert)

Working in teams

The post holder will support and supervise Trainee Healthcare Assistants with their learning and development, also trainee nursing associates, apprentices, health coaches and those new to care roles.

The postholder will supervise volunteers If this is required.

Improving safety and quality of care

Nursing associates role in maintaining and improving care, will be to support BHF with research and evaluation, quality improvement strategies.

The Nursing associate will participate in audit, health and safety activities, as discussed with Head of Nursing

Nursing associate will escalate any risks or safeguarding concerns to the GPN and follow the policy for safeguarding.

The post holder will escalate any specific risk / concerns to the staff on duty and act according to local policy

Contributing to integrated care

Post holder will refer to agencies as necessary e.g. Community Nursing / Social Prescribing, receive referrals to or from, or co-ordinate care with the GP

The posts responsibilities are to refer for blood tests as delegated by GPN - Tasks relating to these teams or agencies/ wound care / phlebotomy etc.

Responsibilities are the safe transfer of care between professionals, settings or services using SystmOne

The post holder may work across the PCN in other roles if employed such as Health Coach

Responsibility for Information Resources

Record care undertaken in the SystmOne notes.

Maintain notes that are accurate.

Ensure patient records comply with issues relating to confidentiality and safe storage.

Be able to use the NHS.NET email system in line with BHF local policy and procedure.

Zero Tolerance

Ensure that active de-escalation and diffusion techniques are used to minimise risk of physical aggression, during which the safety of patients, self and other members of the team are considered a priority.

Emergencies

Respond to emergency situations as they occur and be prepared to act as support where necessary.

Carry out physical / psychological care for service users e.g., enhanced observation and follow the required reporting protocols.
