Cwm Taf Morgannwg University Health Board are offering an exciting and challenging opportunity to work as a Nurse Assessor.

We are looking for a professional who is reliable, flexible and self-motivated who can work as part of the Nurse Assessor Team across Cwm Taf Morgannwg  UHB and has a working knowledge of both the Continuing Health Care Framework and NHS Funded Nursing Care where care is provided via the Independent Sector.

You will undertake nursing assessments in care home settings throughout the Cwm Taf Morgannwg footprint  to establish eligibility for NHS Funded Nursing Care and also co-ordinate and undertake Multi Agency Disciplinary Meetings to establish an individual's eligibility for Continuing NHS Health Care.

Main duties of the job

The post holder will provide comprehensive assessments supporting the Continuing Health Care and NHS Funded Nursing Care process.  The post holder will work as part of a multi-disciplinary team, acting as a resource to provide nursing advice in relation to appropriate placement and management.

The post holder will develop and maintain a collaborative approach to the fair application of both NHS Funded Nursing Care and Continuing Health Care criteria.  The post holder will encourage joint working with partnership agencies and actively work towards improving outcomes for patients via the process, and provide accurate advice and information to Lead Nurse/Co-ordinator, families etc regarding the outcome of the assessment process.

The post holder will support the Lead Nurse for CHC & NHS FNC and the Senior Nurse Assessor in the future development of the Nursing Team.   This will also include the facilitation of integrated working with colleagues in Local Authorities and 3rd sector organisations to support individuals.
