Detailed job description and main responsibilities

The Neighbourhood Health Commissioning Lead provides senior operational and commissioning leadership for the development and implementation of the Neighbourhood Health Delivery Model within a defined geography. The post translates ICB strategy and population health priorities into deliverable programmes, working across organisational and professional boundaries to develop integrated, person-centred services and mature multidisciplinary neighbourhood teams.

The role works collaboratively with NHS providers, local authorities, Primary Care Networks, community partners and the VCSEF sector. It has a significant role in joint commissioning, Better Care Fund arrangements, performance and assurance, financial and resource management, health inequalities, patient flow and UEC pressures, and the development of sustainable models of care.

  • Neighbourhood Health: Lead the development and implementation of the Neighbourhood Health Delivery Model, aligned with strategic and local population health priorities.
  • Commissioning: Lead commissioning approaches for multidisciplinary neighbourhood teams, including specifications, market management and procurement.
  • Service transformation: Translate strategic ambitions into operational delivery, supporting integrated, preventative and person-centred models of care.
  • Better Care Fund: Play a leading role in the co-production, review and monitoring of the BCF with local authority and wider partners, ensuring investment delivers defined outcomes and meets reporting requirements.
  • Joint funding: Support oversight of joint funding arrangements, including Section 75/76 and delegation agreements, ensuring appropriate governance and supporting integrated delivery.
  • Partnership working: Build effective relationships across NHS providers, local authorities, primary care, neighbourhood teams, VCSEF organisations and community representatives.
  • Performance and assurance: Support performance management, assurance and improvement, monitoring outcomes, reducing unwarranted variation and escalating risks appropriately.
  • Population health and inequalities: Use commissioning and data-driven interventions to improve healthy life expectancy, reduce inequalities and improve access for underserved communities.
  • Financial leadership: Manage budgets and resources within delegated limits, work closely with finance colleagues, identify efficiency opportunities and support value for money and sustainability.
  • Operational resilience: Provide senior support for patient flow and UEC demand, including winter planning and responding to significant local flow challenges.
  • Governance: Maintain appropriate governance and compliance, contribute to policies and procedures, and provide complex reports and assurance information to senior leadership.
  • Leadership and management: Provide visible leadership, manage teams/programmes, support workforce development and foster collaboration, accountability, innovation and continuous improvement.
  • System representation: Represent the neighbourhood agenda at system forums, delivery boards and partnership groups
