Job overview

Sussex Provider Collaborative 12‑month Fixed Term / Secondment | Hybrid | Sussex‑wide

Are you passionate about improving health outcomes, reducing inequalities, and delivering change that makes a real difference to people and communities? Do you thrive in complex, cross‑organisational environments where collaboration, innovation and partnership are essential? If so, this is an exciting opportunity to join the Sussex Provider Collaborative as a Neighbourhood Delivery Manager.

About Us

The Sussex Provider Collaborative (SPC) is entering an exciting new phase of transformation. As an “all‑in” partnership of NHS organisations across Primary Care, Mental Health, Learning Disability, Community, Acute and Ambulance services, we are working collectively to deliver change at scale, improving quality, care, and outcomes for our population.

Our Programme Delivery Team (PDT) drives this transformation. We lead neighbourhood, acute and system‑wide programmes that bring together expertise from across Sussex providers to deliver measurable, meaningful improvements.

This is a rare chance to work across organisational boundaries as part of a truly system‑first, provider‑agnostic team.

Main duties of the job

The Role As a Neighbourhood Delivery Manager, you will play a key role in translating Neighbourhood Alliance priorities into delivery at Place level. Working across organisational boundaries, you will support the design, implementation and evaluation of local care models that improve outcomes and experience.

You will:

  • Lead delivery of Neighbourhood Alliance workstreams across Sussex.
  • Develop and manage programme plans delivering benefits.
  • Work with ICTs to co‑design and scale neighbourhood models.
  • Monitor performance, risks and benefits, feeding into governance.
  • Build partnerships across PCNs, Local Authorities, and health, care and voluntary sectors.
  • Support ICT Integrators and promote system‑first working.

This is an opportunity to contribute to integrated care transformation at pace and scale.

About You You will be an experienced programme or project professional, skilled in delivery, influencing and partnership working. You will work autonomously, resolve barriers, and focus on improving outcomes.

You will bring:

  • Experience in health, care or multi‑agency settings.
  • Strong programme management skills.
  • Excellent communication and engagement skills.
  • A collaborative, inclusive mindset.
  • Resilience and adaptability.

Why Join Us? A supportive environment, the chance to shape system transformation, flexible hybrid working across Sussex, and an opportunity to make a real difference.

Travel across Sussex will be required.
