Band 5 RMN / OT / Social Work Professional

Exeter City Core Community Mental Health Team (CMHT) Full-Time Substantive Post (37.5 hours per week) Flexible / Hybrid Working Available

We are seeking enthusiastic and compassionate Band 5 Registered Mental Health Nurses (RMNs), Occupational Therapists (OTs), or Social Work professionals to join our Exeter City Core CMHTa team known for its dynamic, innovative, and supportive culture.

Main duties of the job

Main Duties & Responsibilities

  • Deliver high-quality, person-centred care to adults with complex mental health needs in the community.
  • Work collaboratively within a multidisciplinary team to develop and implement care plans.
  • Conduct assessments, risk evaluations, and reviews in line with clinical governance and safeguarding protocols.
  • Provide therapeutic interventions and recovery-focused support.
  • Liaise with service users, carers, and external agencies to ensure continuity of care.
  • Maintain accurate and timely clinical documentation.
  • Participate in team meetings, supervision, and ongoing professional development.

Job responsibilities

The post holder will deliver high quality mental health services, The role involves managing a defined caseload. The focus will be providing continuous assessment and treatment to people with mental health difficulties and their carers. This will include key working and care management functions, liaison with statutory and non-statutory organisations. The post holder will be an effective member of the multi-disciplinary team.
