We have an exciting opportunity for a Band 6 Mental Health Practitioner to join our Rough Sleeper Treatment and Recovery Team in North Northamptonshire. NHFT and North Northants Council have successfully developed the role, working closely together and sharing a vision for how Rough Sleepers can access support with their Mental Health.

Main duties of the job

Liaise and communicate effectively with statutory and non-statutory mental health agencies.

To maintain protocols for effective communication between both local mental health services, 3rd sector partners and other interested organisations.

To oversee the specialised risk and mental health assessments of individuals engaged with the Rough Sleeping Team to inform recommendations for their care/treatment.

To promote and develop effective working relationships with key agencies.

To act as a focal point for the identification and planning of aftercare for individuals.

Improve the experience of patients with mental health illnesses who receive support from the Rough Sleeping Team.

Provide specialist mental health perspective to the multi-disciplinary team members regarding assessment, intervention, management and safety planning.

Contribute to support, training and skill development for individuals and groups in mental health awareness.

To offer a range of interventions for individuals with a wide range of mental health difficulties

There will be an expectation that out of hours support will be provided working alongside the Rough Sleeping Team during night time outreach across North Northants. This will involve attending, carrying out assessments and treatment in urban, rural and remote locations.

Detailed job description and main responsibilities

To carry out detailed mental health assessments of individuals, addressing the holistic aspects of their health needs.

To ensure implementation of a need based response system that seeks to build on the Hope, Control and Opportunity of service users

Managing own small case load of service users through focused interventions and working alongside Individuals to help navigate systems, leading by example to deliver against desired outcomes.

To support and oversee the formulation and implementation of appropriate care plans/packages of care for the individual and referrals to ensure that their mental health care is as seamless as possible.

To assist in the maintenance of appropriate use of the Mental Health Act where appropriate.

To promote and facilitate co-production with the service user in all processes and interventions throughout the services within this position.

To ensure at all times that the team involve the service user in the assessment process, to ensure that all care pathways are individualised in their form.

To work in collaboration with acute hospitals, external mental health agencies, and Police in order to address mental health needs of the individual.

To work collaboratively with colleagues throughout the crisis care pathway and the wider Specialty Services.

Maintenance of up-to-date clinical records in line with Trust policy, and professional registration standards.
