Job overview

We are seeking a motivated and compassionate Band 6 Mental Health Practitioner to join our dynamic Locality Mental Health Team, providing high‑quality, recovery‑focused care to adults with complex and enduring mental health needs. You will work both autonomously and as part of a supportive multidisciplinary team, delivering evidence‑based interventions that promote safety, wellbeing, and independence.

As a key member of the team, you will hold a defined caseload, act as a lead HCP and deputise for the team manager when required. You will play an active role in assessment, formulation, risk management, and collaborative care planning, ensuring that service users and carers are at the centre of decision‑making.

You will be an experienced and confident practitioner with:

  • Professional registration (RMN, Social Worker, or Occupational Therapist).
  • Strong clinical assessment, formulation, and risk‑management skills.
  • Experience working with adults with complex mental health needs in community or acute settings.
  • A commitment to trauma‑informed, recovery‑focused, and co‑produced approaches.
  • Excellent communication skills and the ability to work collaboratively within a multidisciplinary team.
  • Confidence in managing competing demands in a fast‑paced community environment.
  • Motivation to contribute to service development, quality improvement, and positive team culture.

Main duties of the job

Comprehensive assessment — Undertake biopsychosocial assessments, formulation, and risk assessments for adults referred to the service.

Case management — Manage an allocated caseload, developing person‑centred care plans in line with the LMHT SOP and Framework.

Risk management — Lead on dynamic risk assessment, crisis planning, and safety interventions.

Therapeutic interventions — Deliver evidence‑based interventions, including psychological, psychosocial, and recovery‑focused approaches.

Duty work — Participate in the team’s duty rota, responding to urgent referrals and supporting crisis presentations.

Multidisciplinary collaboration — Work closely with psychiatry, psychology, social care, primary care, and voluntary sector partners.

Clinical documentation — Maintain accurate, timely, and professional records in line with Trust and NMC/HCPC standards.

Service development — Contribute to quality improvement, transformation initiatives, and trauma‑informed, recovery‑oriented practice.

Supervision and support — Provide supervision, mentorship, and support to junior staff and students.

Safeguarding — Uphold safeguarding responsibilities for adults and children, working within statutory frameworks.
