An exciting and unique opportunity has arisen to be a part of the Rehab Case Management Team working within their Rehab Access Function.
In this role, you will be responsible for developing and delivering tailored assessments for individuals undergoing rehabilitation, both in community and inpatient settings. You will manage a caseload of patients within our Inpatient Rehabilitation Services, ensuring high-quality, patient-centred care.
In addition to your clinical responsibilities, you may be required to step in as a deputy for the Clinical Lead during periods of absence, such as annual leave or sickness. This will involve serving as the primary point of contact for both internal and external agencies related to the Rehab Access function, ensuring smooth operations and continuity of care.
You will manage a caseload of individuals across both local and national inpatient rehabilitation services, including Level 1 services within the locality and specialist placements in Level 2 services.
Your primary responsibility will be overseeing and reviewing the treatment progress of patients in hospital, ensuring that their rehabilitation goals are met effectively and on schedule. This will involve attending all scheduled meetings, conducting face-to-face reviews every 12 weeks, and maintaining regular communication with families and carers to ensure continuity of care and support and ensuring all required documentation is completed appropriately.
