Job overview

Fenland Adult Locality Team is based in Wisbech, Cambridgeshire. We are a small team and work in a rural catchment area.

The role will take on the care co-ordination for clients and provide leadership to support to others, to ensure that full assessment of individual care needs are undertaken, that care is planned, implemented and reviewed on a regular basis. Dialog + is supporting patients care plans and recovery goals which you will manage for patients on your caseload.

You will participate on the duty rota and be a member of the MDT.

Main duties of the job

  • To participate on the duty rota.
  • To act as a role model and promote effective teamwork across the service to meet individual client needs.
  • To ensure that the CPA and Mental Health Act is adhered to at all times, and to work within the boundaries set out by the Trust’ Policies and Procedures.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.   • The post holder will provide a good role model, develop and maintain good systems of communication, be able to communicate with a wide range of people in a range of situations
  • Reporting verbally and in writing with regards to clients progress, assessments and care planning to the wider multi-disciplinary team
  • Maintain accurate and complete records
  • Discussing with client and their families / carers regarding care planning, assessments and treatment
  • Be able to acknowledge the barriers to effective communication and adapt communication style in response
  • The post holder will be involved in self-development and contribute to the development of others.
  • Engaging in reflective practice and implement evidence-base to practice, linking to appraisal standards.
  • Identification of individual development needs and accessing learning opportunities available.
  • Encouraging and enabling others to develop and apply knowledge and skills, by acting as a supervisor and mentor.

  • Registered Nurse Mental Health (RNMH)
  • Current NMC Registration

  • Knowledge of physical healthcare needs
  • Awareness of safeguarding issues and responsibilities under relevant legislation
